Care plan meetings are meetings in which your loved one’s care team assembles to assess your loved one’s care, discuss their recovery progress, and make sure all is well.
How frequent are care plan meetings?
In a skilled nursing facility, care plan meetings happen once per week. Care plan meetings will occur in your loved one’s room and there will be a representative from the interdisciplinary team in attendance.
This means a care plan meeting will include representatives from each department:
- Physical and/or occupational therapists
- Nursing staff
- Social workers
- Activities
What gets discussed during a care plan meeting?
During the care plan meeting, attendees will discuss:
- Your loved one’s progress
- The plan in place to help your loved one
- Your loved one’s discharge plan, including where your loved one will go when they’re discharged and whether the environment is safe.
One of the main objectives of care plan meetings are to ensure your loved one will be safe upon discharge.
